Disclosure Agreement: Please review and sign below to acknowledge your understanding and agreement to the event terms and policies.
CIOFF® USA Youth Folk Dance Workshop – April 11, 2026 | Provo, Utah
1. Definitions: “Released Parties” includes CIOFF® USA, CIOFF® USA Youth, CIOFF® International, their officers, directors, agents, employees, volunteers, instructors, contractors, representatives, sponsors, affiliated organizations, venue providers, and associated partners. “Event” refers to the CIOFF® USA Youth Folk Dance Workshop and all related activities.
2. Assumption of Risk: Participation in dance and physical activity involves inherent risks, including injury, illness, falls, collisions, equipment-related injury, and medical emergencies. I knowingly and voluntarily assume full responsibility for all risks of injury, loss, damage, or death resulting from participation.
3. Release and Waiver of Liability: To the fullest extent permitted by law, I release, waive, discharge, and covenant not to sue the Released Parties for any claims arising from participation in the Event, including claims based on ordinary negligence, premises liability, or acts of volunteers or instructors.
4. Indemnification: I agree to indemnify and hold harmless the Released Parties from any claims, damages, liabilities, or legal fees arising from participation in the Event.
5. Medical Authorization: I certify that I (or my minor child) am physically able to participate. I authorize emergency medical treatment if necessary and accept full financial responsibility for any medical services provided.
6. Insurance Acknowledgment I understand that CIOFF® USA does not provide medical or accident insurance coverage for participants.
7. Photo & Media Release I grant permission for photographs, video, and recordings to be used for promotional, archival, and educational purposes without compensation.
8. Code of Conduct: Participants must follow safety guidelines and staff directions. CIOFF® USA reserves the right to remove disruptive participants without refund.
9. Severability: If any portion of this Agreement is deemed unenforceable, the remaining provisions shall remain in effect.
10. Governing Law: This Agreement shall be governed by the laws of the State of Utah. Legal actions shall be brought exclusively in Utah County, Utah.
